I think I can get this back to "good", but sadly will lose the decal in the process...
Splice in a new piece of matching maple, re-shape the peghead, paint the face to match whatever color the body is going to be, and layer the back of the peghead with a thin matching veneer of maple to hide the hole repairs...
I have an original period-correct decal....

The body is a mess...
It almost looks like poly over the original finish??

I did some stupid things to guitars when I was a kid, but...... :fp:

I think i would leave the headstock as is, or at least just glue some maple on it (but would'nt it break easily since it's not all the same wood? It doesn't seem as stable)...
Good luck with the project!

Edit: (maybe you're not going to change the tuners to the glued part)

Yeah, ditto...

Yeah, there's two problem areas...
Hard to describe, but there are two spots where wood needs to be built up so that a vintage outline can be achieved...
Both can be disguised by peghead face paint, and peghead back veneer...
Here's a veneer on a similar '62 Jazzmaster project, hiding a 3/4" "cabin hole"...

And the front...
You can see the outline of the hole that showed when I checked the Sonic Blue...
Oh well...

It looks like a freaking banjo.

I repaired my XII neck with a veneer. It was even worse (cut in half). Perfection moved on many years ago for this thing. But it's solid and sounds great. A seam line appeared when I put the tuners in. A veneer is cosmetic, but also acts a skin to bind the it all together. The other trick is to build a good base layer with Zpoxy and then paint. That could also fix the cabin hole in the other neck. At first blush it sounds like Pet Sematary, but every Gibson has a slice of holly on the headstock, and many have a maple cap on the body. We all want steak, but will take a burger wrapped in bacon.
