faulty ground connection.
the reason why the noise goes away when you touch it is because your hand is grounding the guitar.

I think it might just be the jack or cable since it appears that everything has the grounding problem?

At a point I was renting out a unit who had a faulty ground in the building wiring and a bunch of weird problems would arise, it's the landlords duty to fix that.
